BOARD MEMBER
Sam Hazen has served on the board since 2017. She currently serves as Treasurer. Sam holds a Bachelor Degree in Psychology from Bemidji State and a Social Work Certificate from UW Stout. She serves as the Emergency Services Social Worker for Polk County Behavioral Health Department. Sam loves jigsaw puzzles, her dogs and spending quality time with friends and family.
The mental health and substance use needs of the county and beyond are important to Sam. She is interested in reducing stigma, suicide and barriers to services. Sam lives in Polk County with her dogs Maya and Reggie.
